Directors: Students selected to attend Honor Band need to print and complete the PARENTAL CONSENT FORM (below), have their parents sign it, and return it to you. The Parental Consent Form, Registration Checklist, and payment should be postmarked by April 16, 2010. Only one check per school please. Band Boosters/Directors, please make checks payable to Gray Echelon.
